Welcome to The Harborage
The Harborage is a 245-bed nursing home providing outstanding, long-term care in a positive, compassionate, and unique setting.
Located right on the Hudson River and adjacent to Palisades Medical Center, The Harborage offers spectacular views of Manhattan along with the peace of mind that comes from being so close to acute care if and when it's needed. And the opposite is true, too — Palisades Medical Center residents in need of continued care can find it at The Harborage.

In each section of The Harborage – Long Term Care, Subacute Care/Rehabilitation and Ventilation Therapy – residents receive consistent, comprehensive care and services from a dedicated Care Team. Care Teams are assembled based on the type of care required as well as individual needs. But we're just as committed to other aspects of our residents' lives.

We offer private and semi-private accommodations. All rooms are spacious and can be personalized with your favorite chair, family photos, and other valued possessions. Residents gather for meals in our dining facilities, which include a formal dining room for entertaining family and friends. Social activities take place in lounges and multi-purpose rooms.
We prepare your meals, do laundry, and help with bathing, grooming, and hairdressing. Our complete housekeeping services are designed to pamper you and relieve you of having to worry about these things.
At The Harborage, food service is provided by Sodexo, the leader in integrated food service and facilities management in the U.S., Canada, and Mexico. Sodexo is in business to create and offer services that contribute to a more pleasant way of life for people wherever and whenever they come together. One of the "World's Most Admired Companies" according to Fortune Magazine, Sodexo is dedicated to making every day a better day for residents at The Harborage.
Our residents provide input and participate in regular menu planning meetings, and choose what they want to eat from 30 selections for lunch, 30 selections for dinner, and at least 10 selections for breakfast — every day. Choices include daily specials as well as food that honors culture and religion.
There's always plenty to do at The Harborage! Our recreational therapist and staff create engaging activities for every day and every evening of the week, every week of the year. And because of our unique location, we're able to offer regularly scheduled outings that take advantage of what both the vibrant local community and the general metropolitan area have to offer.

Residents can also take part in religious services at our Interfaith Chapel. Staff and volunteers visit with residents frequently, and family members are welcome every day of the year.
Long Term Care
Long Term Care is personalized care at The Harborage. When you're admitted, you'll receive an evaluation by nursing, dietary, social services, rehabilitation and recreational therapy professionals. This interdisciplinary approach is used to develop a care plan that meets your specific needs.
We encourage every resident to develop and maintain as much independence as possible, and your dignity and comfort is our top priority. We value your individual right to privacy and respect your decisions regarding treatment options.
Long Term Care services include:
- IV antibiotic therapy
- Wound care — including surgical, pressure wounds, diabetic and vascular wounds
- Tube feeding
- O2 therapy
- Trach care
- Respiratory services
- Restorative nursing — supervision, direction, assistance, training or retraining in all phases of daily living to promote independence or growth and to develop or restore optimal functions of the individual. This may include active and passive range-of-motion, bowel and bladder re-training.

- Catheter care
- Pacemaker monitoring
- Terminal illness management including hospice and palliative care
- Pain management
- Discharge planning
- Ongoing assessment for individual care planning by registered nurses
- Treatment care
At The Harborage, subacute and rehabilitation care combines the high-tech resources of a hospital with the operation of a skilled nursing facility. We help residents overcome and adapt to physical and neurological challenges brought about by stroke, Parkinson's disease, orthopedic injuries, chronic arthritis, and pulmonary conditions. Wound care services are also available for infections and vascular ulcers.
Our doctors, therapists, nurses and other health professionals work together in teams, assembled according to individual needs. We provide each resident with the most effective and efficient care possible, because our goal is to help them return home as quickly as possible.
And when residents are able to return home, our social services staff assists with the transition, including helping to make special arrangements and educating families on how to provide proper care.

Rehabilitation is any process that helps residents achieve the highest possible quality of life and health possible, based on their individual needs. The Harborage is a contracted provider of Genesis Rehabilitation Services (GRS), a division of Genesis Health Care. Why is that important to you? GRS is one of the nation's leading providers of physical, occupational, and speech therapy services. It touches the lives of more than 80,000 residents a year in more than 23 states, entirely focused on clinical excellence and continuous staff development.
Giving people the "skills for the job of living" — that's what our occupational therapists do every day. We determine your present fitness level, and guide you to rebuild the skills you need to improve mobility and independence.

The ability for human communication is one of the most important gifts a speech-language pathologist gives to our residents. Our speech pathologists offer instruction to help residents improve speech patterns or learn new forms of communication.

Depending on personal preference and level of fitness, residents take part in a variety of programs that stress social interaction while building strength and mobility.
The ventilation unit is located in separate facilities at the Harborage. The unit features thoroughly modern accommodations and advanced medical equipment, as well as a number of safeguards to ensure continuity of care in emergencies. Resident rooms surround a central nursing station, dining area, recreation room and physical therapy center. Private areas are available for family visits and home care training. Therapeutic equipment includes piped-in oxygen, supplementary tanks, suction equipment, medical outlets, and emergency electrical power. Respiratory specialists are available 24 hours a day to care for residents and manage emergencies.
Upon admission, you will receive an evaluation to determine your potential to breathe without a ventilator. Specialists work closely with residents to decrease dependency on life support systems in a safe, encouraging environment. Along with physical, occupational, and speech therapy, residents take part in a variety of recreational programs to improve morale. Individuals and families are offered counseling to cope with the emotional aspects of chronic illness.
